你早上刷牙的时候,会不会顺便刮一下舌头?
这几年,刮舌头在网上越来越火。
一个小小的金属片,弯成"U"字形,放在舌头后面,往前一拉,舌头上那层白白的东西,就被刮了下来。
那层东西叫舌苔,里面有食物的残渣、死掉的细胞,还有大量的细菌。
刮舌头其实很老了。
印度的传统医学阿育吠陀,很久以前就有刮舌头的习惯。
现在,它变成了一门生意,网上卖得很好。
那么,它到底有没有用?
先说口臭。
口臭的主要原因,是舌头后面的细菌,会产生一种带硫的气体,叫硫化物,闻起来像坏鸡蛋。
二零零四年,巴西圣保罗大学做了一个小实验。
他们请了10个人,先用牙刷刷舌头,这种气体少了45%。
同样的人,换成刮舌头的工具,气体少了75%。
结论很简单:去口臭,刮比刷有效。
不过,这个实验只有10个人。
二零零六年,有学者把相关的研究放在一起,做了一个系统综述,结果只找到2个小研究。
他们的看法是:刮舌头对口臭有帮助,但是效果比较短,而且证据不算多。
说到这里,好像就可以下结论了:想去口臭,可以刮。
但是,事情还有另一面。
舌头上的细菌,不全是"坏人"。
你吃菠菜、甜菜根这样的蔬菜,里面有很多硝酸盐。
硝酸盐被吃进去以后,一部分会回到嘴里。
舌头上的一些细菌,能把它变成另一种东西,最后在身体里变成一氧化氮。
一氧化氮能让血管放松,血压就会低一点。
也就是说,舌头上的一部分细菌,其实在帮你降血压。
那如果把它们杀掉,会怎么样?
二零一九年,有一个研究,请了26个人,每天用2次很强的杀菌漱口水,叫氯己定。
一个星期以后,他们的平均血压,从113升到了115。
数字不大,但是方向很清楚:口里的细菌被大量杀掉,血压就上去了。
那刮舌头,会不会有同样的问题?
同一个研究给了一个让人放心的答案。
平时每天刮舌头的人,舌头上那些"会帮忙"的细菌,比例还更高。
原因可能是:刮舌头只是把舌苔刮走,是物理的办法,不会把细菌全部杀死;刮完以后,有用的细菌还会长回来。
研究者的建议是:刮舌头,加上多吃蔬菜,可能对血压有好处。
真正要小心的,是天天用强力杀菌的漱口水。
所以,回到开头的问题:你要不要刮舌头?
如果你在意口臭,刮一刮没有坏处,但是别期待它能管一整天。
如果口臭一直很严重,那可能是牙齿或者胃的问题,应该去看医生。

When you brush your teeth in the morning, do you scrape your tongue while you're at it?

In the last few years, tongue scraping has been getting more and more popular online.

A small metal strip bent into a U shape: put it at the back of your tongue, pull forward, and the white layer on your tongue gets scraped off.

That layer is called tongue coating; it holds bits of food, dead cells, and huge numbers of bacteria.

Tongue scraping is actually very old.

India's traditional medicine, Ayurveda, had a tongue-scraping habit long ago.

Now it has become a business, and it sells very well online.

So does it actually work?

Let's start with bad breath.

The main cause of bad breath is bacteria at the back of the tongue, which produce a sulfur gas called sulfur compounds that smells like rotten eggs.

In 2004, the University of São Paulo in Brazil ran a small experiment.

They asked 10 people to brush their tongues with a toothbrush first, and the gas dropped by 45%.

The same people then switched to a tongue-scraping tool, and the gas dropped by 75%.

The conclusion is simple: for bad breath, scraping works better than brushing.

However, the experiment had only 10 people.

In 2006, scholars put the relevant studies together in a systematic review, and found only 2 small studies.

Their view: tongue scraping helps with bad breath, but the effect is fairly short, and there isn't much evidence.

At this point, it seems we can conclude: if you want to fight bad breath, you can scrape.

But there's another side to this.

Not all the bacteria on your tongue are 'bad guys.'

Vegetables like spinach and beetroot contain lots of nitrate.

After you eat nitrate, part of it comes back into your mouth.

Some bacteria on your tongue can turn it into something else, which eventually becomes nitric oxide in your body.

Nitric oxide relaxes your blood vessels, so your blood pressure goes down a bit.

In other words, some of the bacteria on your tongue are actually helping lower your blood pressure.

So what happens if you kill them?

In 2019, a study asked 26 people to use a very strong antibacterial mouthwash called chlorhexidine twice a day.

After a week, their average blood pressure rose from 113 to 115.

Not a big number, but the direction is clear: kill off lots of mouth bacteria, and blood pressure goes up.

So does tongue scraping have the same problem?

The same study gave a reassuring answer.

People who scraped their tongues every day had a higher share of the 'helpful' bacteria.

The likely reason: scraping only removes the coating; it's a physical method that doesn't kill all the bacteria, and afterwards the useful bacteria grow back.

The researchers' suggestion: scraping your tongue plus eating more vegetables may be good for your blood pressure.

What you really need to be careful about is using strong antibacterial mouthwash every day.

So, back to the opening question: should you scrape your tongue?

If you care about bad breath, scraping does no harm, but don't expect it to last all day.

If your bad breath is always severe, it may be a tooth or stomach problem, and you should see a doctor.
