It describes events coming into being — especially incidents, accidents, disputes, wars, disasters, or changes (发生事故 an accident happens, 发生战争 war breaks out, 发生变化 a change occurs). It is register-neutral, leaning slightly formal, and appears constantly in news and narration. A key difference from English 'happen': 发生 is normally used for the event as subject placed after it, not for personal experiences — you say 发生了一件事 ('something happened') but you would not say 'what happened to you' with 发生 in casual speech (that is 你怎么了).
It also cannot take a person as its direct object. It commonly pairs with the completed-action particle 了.
