How to Think About It
换成 literally means 'swap into' — you're swapping the listener into the situation that was just described. 是你 names them as the new occupant of that scenario. The whole phrase only works after a context has been set up; you can't open a conversation with it. It's a pivot from 'here's what happened to X' to 'now imagine it happened to you.'
